Prescription / Referral:

Elite Physical Therapy does not require a physician prescription or referral in order to schedule an appointment. However, your personal health insurance company may require this information for payment. You may want to contact your insurance company prior to scheduling to inquire about specific out-patient physical therapy benefits.

First Visit:

Please arrive 10 minutes before your scheduled appointment time to fill out the necessary paperwork. To save time, you can download our forms and bring the completed forms to your first visit. New patients must fill out the Patient Information, Medical Information, Consultation & Treatment, and Cancellation Agreement forms. In addition, please complete any forms applicable to your condition.

What to bring:

Please bring your physician prescription/referral, license, insurance information (cards, workers comp/PIP information.) and any pertinent medical information (MRI/X-ray reports, medication lists, post-operative reports/protocols).

How long will the evaluation/treatment session last?

All evaluations and treatment sessions are one hour one-on-one with your licensed physical therapist.

Which forms do I need to fill out?

All new patients must fill out all forms for new patients. Medicare patients must fill out one functional measure questionnaire. Please fill out the form that is most relevant to your injury/condition.

What if I don't see my insurance company on the list?

If you don't see your insurance company on the list, please call our office to see if we participate with your insurance company.
